Our health information exchange platform, the Hudson Valley Health Information Exchange (HVHIE), provides several services designed to unify separately operating care processes and data silos into a community-wide asset of integrated systems and information.
Health Data Routing & Delivery
The Community Registry & Router or CRR serves as an index into community data or locating information in a federated system. This allows record location services across disparate sources. Additionally, the CRR routes data to and from the proper locations when requested to deliver information across the network.
Identity Normalization
The HVHIE performs a service allowing the matching of person identities across independent organizations and systems, commonly called a master person (or patient) index (MPI). This is key for tracking patients across different healthcare organizations and care settings. It is also useful for reconciling physician identities across multiple affiliations. The MedAllies offering automatically correlates, tracks, and translates identity information among connected organizations and systems.
Health Data Normalization
The HVHIE performs a service enabling the assignment of equivalence and the highlighting of distinction between medical concepts that are diversely represented among unaffiliated organizations and systems. This allows direct comparison of data across institutions which facilitates proper interpretation of information between contexts, allows for seamless integration of data across sources, and enables comparative analysis such as is needed in pay-for-performance programs.
Quality Reporting
The HVHIE enables quality reporting using national metrics developed by leading healthcare industry consortium and authorities. An eclectic group of CCHIT-certified electronic health record systems are able to calculate and share quality metrics for community benchmarking and voluntary transmission for pay for performance or compliance purposes.
